Maandig became engaged to Trent Reznor in April 2009, and they married on October 17, 2009 at a private ceremony in Reznor's home. Maandig gave birth to a son, Lazarus Echo, in October 2010 and they are currently expecting another child.

In 2010, she and her husband Reznor formed the group How to Destroy Angels, along with long-time Reznor collaborator Atticus Ross. They have so far released a debut eponymous EP, with a full album scheduled for release in 2012.
